Prepare the pan: Line an 8x8-inch baking pan with parchment paper, leaving some overhang for easy removal later.
Melt the ingredients: In a medium saucepan over low heat, combine chocolate chips, condensed milk, and butter. Stir constantly until fully melted and smooth.
Add flavorings: Remove from heat and stir in vanilla extract and salt. If using nuts, fold them in now.
Pour into pan: Transfer the mixture to the prepared pan and spread evenly with a spatula.
Chill: Refrigerate for at least 2 hours, or until the fudge is firm.
Cut and serve: Lift the fudge out of the pan using the parchment paper. Cut into squares and enjoy.
